Dear Parent or Guardian,
You are being asked to take a survey. The survey
is being administered by the Utah Education Policy Center on behalf of the Utah
State Office of Education. The purpose is to get your input about your
child’s teacher, principal, and school. Your responses will be completely
anonymous. The survey is about 30 items long will take approximately
10 minutes to complete. It is teacher-specific, so if you have more than
one student at this school please take one survey for each child. All
parents and guardians are invited to take the survey. The survey is

You can access the survey from any
computer. The survey is available in both English and Spanish.
If you do not have a computer, we strongly encourage
you to take the survey at the school (we have a computer set up for you to take
the survey on) or on any public computer, such as those available at a public
library.
